New Patients, Welcome to Sunrise Medical Center!

Our goal is to provide you with the best patient care and service. We invite you to begin your new patient registration now by following these directions:

1. Make an appointment:

Our telephones are answered from 8 a.m. to 11:30 a.m. and from 12:30 p.m. to 4:30 p.m., Monday through Friday.

Please note, if you are attending an appointment with one of our pulmonologists and your insurance company requires you to have a referral from your primary care physician, please be sure the referral arrives to us well prior to your scheduled visit. Appointments will be rescheduled without a referral as no insurance company allows retroactive referrals.

2. Fill out the new patient forms:

Please download and complete the new patient packet and bring it with you to your first appointment. Having this information allows us to provide the healthcare you deserve during that first visit and all subsequent visits.

3. Prepare for your visit:

Internal Medicine Patients
Please bring a list of your current medications and past medical records from your previous primary care doctor. If you do not have former medical records to bring us, we can have you execute a release form for your previous physician to send to us your recent office notes, laboratory results, x-ray results, etc. This is important not only for the continuity of your care but to insure we do not repeat any tests.

Pulmonology Patients
Please bring a list of your current medications. Further, with pulmonary issues your primary care physician or referring physician usually performs an x-ray, CT or MRI to assist in the diagnosis of your condition. You must bring these x-rays or discs with you at the time of the visit.

Without your x-rays or discs, your physician cannot offer an accurate diagnosis or prescribe a proper treatment plan; therefore failure to bring these may result in your appointment being rescheduled.

4. Get directions:

Find a map of the location where you will see your physician and get personalized driving directions.

5. We look forward to seeing you!:

Please arrive at least 30 minutes early if you have not yet filled out these forms. If you have completed forms with you, you only need to check in with the reception desk 15 minutes before your scheduled appointment.
