New Patients, Welcome to Sunrise Medical Center!

Our goal is to provide you with the best patient care and service. We invite you to begin your new patient registration by following the steps below:

1. Make an appointment:

We have 3 locations in the West Valley. Please call your preferred location to schedule an appointment.

Our telephones are answered from 8 a.m. to 11:30 a.m. and 12:30 p.m. to 4:30 p.m., Monday through Friday.

If your insurance requires a referral, it is your responsibility to be aware of this and to obtain the referral from your Primary Care Physician (PCP) and/or your insurance company. While we will always try to be helpful, it is your responsibility to know the details of your insurance policy and whether your insurance requires you to be pre-approved for care. Your appointment may be rescheduled if your referral is not received prior to your visit.

2. Fill out the new patient forms:

Please complete the patient forms and bring them to your appointment.

For easier access, you can upload your completed forms to your Healow account. Simply download the Healow app on your mobile device or click the link below on your computer to create an account. https://healow.com

Our Patient Portal is a secure, convenient, and easy way to access your health information. Search our practice by entering our practice code:  FCFJAA

Once you complete your account registration, you will receive a welcome email with details of your account. Please call our office if you need assistance setting up your account.

3. Prepare for your visit:

Please bring a list of your current medications.

Please bring any sleep medicine records, chest x-rays, CT scans, pulmonary scans on a CD as well as the written report to your appointment. This will ensure that your physician will have the most complete information and prevent duplication of any testing.

Without your chest x-ray, CT scans, or CD at your appointment, your physician cannot accurately diagnosis or plan a customized treatment plan for you. Failure to bring these may result in your appointment being rescheduled.

4. Get directions:

Find a map of the location where you will see your physician and get personalized driving directions.

Please arrive at least 30 minutes early if you have not yet filled out these forms. If you have completed forms with you, you only need to check in with the reception desk 15 minutes before your scheduled appointment.
